Considering the great uncertainty regarding not only the pathogenetic interpretation of vesical-neck obstruction (VNO), but also the anatomical description and definition of the vesical neck (VN), we have retackled the problem, making anatomicohistologic studies in persons of both sexes, ranging from fetus to adult, in normal and pathological conditions, with the GIL VERNET histotopographic serial technique. We can state that VN is not only urinary sphincter, but also a complex neuromuscular system – above all, genital. It has no importance for the retention of urine; in some pathological conditions, however, it can become an obstacle to the bladder’s voidingi.e., VNO. The causes for the first irritation of the spine, producing spasm and contraction, are due to inflammatory, vascular or degenerative problems of the mucosa, submucosa (urethro-cervicitis) or periurethra (periurethritis and chronical prostatitis).
